Q What is a complaint?
A A complaint is an expression of formal discontent or accusation made in a written or verbal form that alleges criminal conduct, misconduct, neglect of duty, corrupt activity, violation of rules or regulations of the Police Department, or other violation of City or State, rules or regulations.
Q Can I talk to a Police supervisor about my complaint?
A YES. You may speak to an employee's immediate supervisor about your complaint for possible resolution. You are still entitled to file a written complaint in the matter.
Q Does the complaint process address guilt or innocence?
A NO. The complaint process is not intended to address an expression of dissatisfaction that SOLELY addresses guilt or innocence. That will be determined by a court of-law. The complaint process will have NO impact upon pending court action or traffic violations in which you may be involved.
Q Is there a penalty for filing a false police report?
A YES. Pennsylvania Criminal Statutes, Chapter 18-906, provides that filing a false report with a law enforcement agency is a criminal offense. Also, the same civil remedies available to citizens are available to police officers.
Q How is my complaint investigated?
A The complaint is reviewed by the Commanding Officer of the Internal Affairs Division. The complaint is assigned for investigation by an IAD investigator. Upon completion of the investigation, a recommendation is made to the Police Commissioner for disposition. The Police Commissioner may concur with the recommendation, modify the disposition or order further investigation.
Q Will I be notified of the disposition?
A YES. You will be notified of the disposition after the investigation is completed and, if desired, you may review the findings with the investigator.
Q What happens if an employee is found to have acted improperly?
A Aside from arrests arising from criminal conduct, an employee may be subjected to the following disciplinary action if found in violation of any regulations:

• He/she may be assigned for additional training and/or counseling
• He/she may be officially reprimanded
• He/she may be suspended from duty

Any disciplinary action against an employee must conform to City of Philadelphia rules, Police Department rules, union contracts in force, State Statutes and local Civil Services rules.
Q Where do I file my complaint?
A You can obtain and file a complaint form at any Police District, City Hall or the Internal Affairs Division at 7790 Dungan Road, Philadelphia, Pa. 19111. You may also download a complaint form via the Internet and mail it to:

This location is accessible from all areas of the city.

From I-95 North: Take Cottman Avenue Exit and go west on Cottman Avenue to the 1300 block, which is Dungan Road and go approximately 5 blocks to 7790, which is on the left side.

From I-76 & Roosevelt Boulevard North: Take Route I (Roosevelt Blvd.) North to Cottman Avenue, go west (left turn) onto Cottman Avenue to the 1300 block, which is Dungan Road. Turn right onto Dungan Road, drive approximately 5 blocks to 7790, which is on the left side.
Q How do I complete the complaint form?
A Include on the complaint form (Form 75-561) your name, address and phone number. Although the Philadelphia Police Department does accept anonymous complaints, if you do not identify yourself, you will not be informed of the disposition of your complaint. Include in your complaint a statement in which you state the facts surrounding your allegation, include the names and address of witnesses, names of officers, and any additional information that would assist in a speedy investigation.
  Information you provide pertaining to complaints
is treated with strict confidence.

Some final thoughts
The Philadelphia Police Department is interested in the welfare of all citizens and in taking action when employees have proven derelict in their duties or are guilty of wrongdoing. If it becomes necessary for you to file a complaint, you can be assured it will be given a fair and thorough investigation.

By the same token, many times officers perform their duties in an outstanding manner, which in most instances is not mentioned or reported. If you have occasion to observe an outstanding act or service by a Philadelphia Police Officer and feel the action should be noted, feel free to tell the officer or their supervisor of your comments. You are also encouraged to send a letter to the Police Commissioner's office expressing your feelings.

Most Philadelphia Police Officers are dedicated individuals who are diligently performing their duties in an effort to better serve you and our community.
