Hazardous highway conditions contribute to many problems for Philadelphia motorists. Potholes are responsible for causing thousands of dollars in damage to vehicles, defective traffic signals contribute to auto accidents and missing street signs disorient motorists and pedestrians alike. Worse, these conditions may cause delays in the arrival of emergency services. Clearly, Philadelphia residents have an interest in reducing the number of such conditions before they cause some of the difficulties we've outlined.

This form is provided so that you can easily inform us of these conditions, allowing us to take the corrective action necessary to rectify the problem. When you complete and submit the report, your information will be transmitted to the Philadelphia Department of Streets Customer Service Division so they can repair or correct the problem.

If you beleive the hazardous condition to pose an immediate threat to life and property Dial 9-1-1.
